FORT HOOD, Texas — Military investigators have blamed pilot error for a 2015 Army helicopter crash at Fort Hood that killed all four crew members on board.

A preliminary report says the pilot was executing a turn, during a Nov. 23 training exercise, when the chopper stalled. The report says the helicopter lost altitude, the tail hit a tree and the UH-60 Black Hawk crashed.

The Killeen Daily Herald reports the review, released Wednesday, does not say which of the crew members was flying when the helicopter crashed in a northeast part of the Central Texas post.

Investigators determined weather was not a factor, although low-light conditions during the late-afternoon crash might have affected the crew's ability to see obstacles.

The final report is pending from the U.S. Army Combat Readiness Center in Alabama.
